Practice Area

Our practice is centered on the areas of Oughtibridge, Worrall and Wharncliffe Side.

We do not accept registrations for patients who live at addresses that are outside the practice boundary area.

How to Register as a patient

Patients can register by visiting the surgery and completing a registration form which is called a GMS1 form. You can also download one by clicking the link:

You do not need Identity Documents, an NHS number or proof of address to register as a patient with us. However, it is helpful if you can provide these as a supporting document as it helps us to request the transfer of your medical record from your current GP practice, it also helps us to confirm if you live within our practice boundary.

New Patient Medicals

All patients over the age of five are asked to see the practice nurse for a health check as soon as possible after registration. This allows us to discuss your medical history and advise on current healthcare. Background information is also useful should an emergency arise before your full medical records have been transferred to this practice.

Change of Details

Please inform us immediately if you change your address or telephone number (including ex-directory numbers). Please let us know if you change your mobile number as it is important that we have an up to date number for you.
